If i had to guess, i think there is a lot of shame involved. Who wants to be on gub gibs? Who wants to admit to themselves they're that fucked up? Imagine the inner turmoil of wanting to be normal but having these disorders or whatever holding you back. Its probably a whole process even coming to grips with all of it and getting the required help. Especially if all this guy has is two geriatric parents and a retarded brother in his life.
