Now that I'm thinking about it, having shit hospitals is a really high IQ move. Imagine if hospitals were clean, with comfy beds, and good food everyone would find excuses to go on holiday there instead of working like good serfs they are.
In fact good hospitals could wreck your economy. Because there would be more ill people lining up, raising the need for more comfy hospitals with pretty nurses with large titties spilling from their dress, and more people would feel ill with the need to spend a week or two in a hospital. In the end the whole budget would be spent on hospitals and noone would work and pay taxes.
