The irony of quality control lies in the fact that the most error-prone point in the value chain is often the error-checking mechanism itself.
This creates a trust paradox: we place the greatest confidence in error checkers, believing they 'close the loop.' Yet their failures are the most consequential, because they allow systemic faults to propagate under the illusion of correction.
The most robust quality processes do not bolt quality on as an afterthought. Instead, they weave quality into every stage of the process, making it inseparable from the work itself.
For this reason, debunkers, fact-checkers, and even peer reviewers frequently generate higher levels of error—not lower—as their oversight role can mask or amplify mistakes and agendas rather than resolve them.
