African midwives overwhelmingly deal with mothers-to-be with cliterodectomies, delivery in spite of infibulations (vaginas sewn shut), rape in war with instruments, and other horrors. A woman in most of Africa has already been vaginally traumatized. Of course @AllisonPearson’s Western expectations of respectful, gentle midwifery are contemptible or alien.
